Nursing Narrative ScriptIntroductionNurses play a vital role in the coordination and continuum of care for patients. In terms of coordination, nurses are responsible for ensuring that all members of the health care team are aware of the plan of care and are working together to meet the needs of the patient. In addition, nurses frequently act as a liaison between the patient and their family, as well as between the different members of the health care team. Here, I will discuss effective strategies for collaborating with patients and their families, how change management relates to communication and patient-centered care, the ethics of coordinated care, and how policy affects outcomes.Effective Strategies for Collaborating with Patients and Their FamiliesIn any healthcare setting, it is important to establish a good working relationship with patients and their families. This collaborative relationship is essential for achieving desired health outcomes. There are a number of strategies that can be used to foster collaboration. By taking the time to build a collaborative relationship, healthcare providers can improve patient care and better achieve desired health outcomes.Examples (Drug-Specific Interventions/Cultural Competence Strategies)A growing body of research supports the efficacy of drug-specific educational interventions in reducing medication errors and improving patient safety. When implementing these interventions, it is important to consider the specific needs of the patients and their families. For example, patients who are unfamiliar with the healthcare system may benefit from more comprehensive educational materials, while those who have a limited English proficiency may require language services.

By taking into account the potential consequences of various policy choices, decision-makers can ensure that they craft policies that will improve the lives of those they represent. When crafting policy, lawmakers must take into account the potential implications and consequences of their provisions. Failing to do so could result in unforeseen and undesirable outcomes. For example, a provision that is intended to incentivize investment may instead have the unintended consequence of stifling economic growth. Or a provision that is meant to protect vulnerable populations may unintentionally create new vulnerabilities.ConclusionIn terms of continuum of care, nurses provide continuity of care by following up with patients after they have been discharged from the hospital.
