Agency/Program

EJAF or the Elton John AIDS Foundation has a mission statement centered on eradicating AIDS from the Americas and the Caribbean. They raise funds for evidence-based policies and programs and speak out openly about the harsh reality of those suffering with AIDS and HIV. Becoming the world's biggest HIV grant-makers, it holds to main offices in London and New York. EJAF has raised over $349 million for the prevention and treatment of AIDS/HIV ever since it was founded in 1992. With an estimated $7 million invested in the Americas spread across 120 organizations each year, half of this effort is focused on the Caribbean and the Southern U.S.

The agency in addition to its fundraising efforts, offers a unique perspective on issues, tackling the most difficult problems those with HIV / AIDS face. The organization focuses on sustainability and innovation, communicating regularly to allies and their grantees about any new possible opportunities for impact. One such grant EJAF received recently was for out-patient medical care, diseases and conditions, preventative care. The grant was for $55,000 and was authorized in 2013 with the grant makers being DDG Partners Foundation, Inc. The EJAF used this grant to help low-income with HIV / AIDS and fund preventative care measures.

HIV can be prevented and treated. This is what makes up the main mission statement of EJAF. Through the enabling of dozens of community organizations across Latin America, the Caribbean, and the U.S., EJAF takes on innovative and sustainable work for gay men's health. The foundation also supports advocacy and investment for improvements in HIV testing, health, and treatment for Black communities, tackling common barriers that affect the health of people such as incarceration, poverty, and illiteracy. The foundation also helps over 30,000 people gain access to related harm reduction services as well as clean syringes that helps prevent injection-related HIV. This is what the project will help cover in relation to preventative measures and helping those with low-income receive adequate health care and treatment to prevent the spread of HIV / AIDS.

The Project is use of HIV statistics among low-income people and gay/lesbian populations to demonstrate HIV risk. It will require various types of intervention like condom distribution, prevention counseling, risk reduction workshops, and community awareness events. All of these objectives will fall under a $100,000 budget and will be staffed with paid and unpaid (volunteer) workers. A nurse will be hired to provide the training for volunteers and paid workers to educate people on HIV / AIDS awareness, counseling, and workshops. This grant proposal will highlight effectiveness of using statistics for project implementation as well as how successful the project can be based on previous programs.

Checklist

The project will use several kinds of intervention that are low-cost and will be able to help reduce the spread of HIV / AIDS and promote awareness. The first phase of program development is community outreach. By creating awareness of HIV / AIDS and providing information on where people can find help, it will allow the subsequent phases to reach more people. As people come in for help, they will be assisted via the second phase which is risk reduction counseling, HIV testing and counseling, and condom distribution. By providing access to as well as safe disposal of injection equipment along with the preventative measures, those with HIV / AIDS can help reduce the spread of injection related HIV infections. All of this can be done under one program with a nurse serving as the supervisor and trainer of 3 paid assistants. These assistants will train 6 volunteers and there will be 4 areas and the nurse will be collecting any and all blood samples for HIV testing.

Detailed Budget Justification

The $100,000 budget seems like a large amount, but this program needs this amount for it to run for at least 1 year. The nurse will be paid a salary of $35,000 as he/she will be the one to train the others and collect blood samples for testing. The other 3 workers will be paid roughly $10,000 a year each for part-time work and will do most of the counseling/training/cleaning services. The volunteers will not be paid, however, they will be fed lunch daily and that takes into account costs in the budget. Equipment like clean needles, needle disposal kits, personal protective equipment, rental space, and condoms will use up the rest of the $35,000.

Several departments within CCHP will work together to help create this program. They come from the behavioral health department, donations/sponsorships, substance use services, and primary care. Key personnel will be Lorraine Broomfield, the program supervisor of Multitrack Abstinence Program that will help with guidance in counseling services. The other is Jozefa Cheman, nursing director.

Project/Executive Summary

The mission statement for the clinic is prevention of HIV or human immunodeficiency virus infection including its associated disease and death. The clinic will be located in the CCHP or Center for Comprehensive Health Practice. This is an interdisciplinary family-focused health center that offers quality behavioral health treatment, supportive services, and quality primary care in East Harlem New York. Using the EJAF as inspiration, the clinic will use the $100,000 grant to include additional services such as condom distribution and counseling in order to serve the East Harlem Community. The program will seek to help everyone anyone wishing to participate in the program regardless of ability to pay. Unlike agencies like the EJAF, CCHP provides an early childhood development center, and recent implementation of a Hepatitis C treatment program resulting in curing 2 people of HEP C and educating patients of the danger of contracting HEP C as an HIV / AIDS patient.

HIV / AIDS treatment and prevention is crucial in reducing the number of cases of HIV infection. AS Healthy People 2020 objectives show, HIV has become an epidemic within the United States and has developed into a major public health crisis. With over 1.1 million Americans diagnosed with HIV, the spread of HIV has led to 56,000 new cases each year (). The national HIV / AIDS strategy includes 3 primary objectives which will be the same objectives of CCHP. They are:

1. Reduce the number of HIV infections.

2. Increase access to care, improving health outcomes for those living with HIV.

3. Reduce HIV / AIDS health inequalities.

Many people living with HIV / AIDS are people of low-income who are black and/or homosexual. This program is meant to help those within the area of East Harlem (an area with a population of black and low-income people) get the care they need and deserve.

Background

The clinic that will be responsible for the implementation of the program is Center for Comprehensive Health Practice or CCHP. Founded in 1960 by DR. Richard Brotman, the vision to generate a safe place for families to access comprehensive and quality care sparked a 55-year long journey to provide families with lifetime care. CCHS offers four primary services. They are primary care, substance abuse, child development, and behavioral health. They seek to provide services to low-income individuals and began incorporating interdisciplinary care into its model since the 1970's. This means Counselors and providers recognize the significance of case management needs like vocational services to assistance with substance abuse problems.

Problem Statement

There is a growing number of HIV cases each year, especially among low-income, black, and homosexual individuals. Without medical assistance and counseling, untested individuals will continue to spread HIV. There needs to be a way to treat these individuals and help spread awareness.

Goals and Objectives

1. Reduce the number of HIV infections.

2. Increase access to care, improving health outcomes for those living with HIV.

3. Reduce HIV / AIDS health inequalities.

4. Use of HIV statistics among low-income people and gay/lesbian populations to demonstrate HIV risk.

5. Evaluate program effectiveness monthly.

6. To assist low-income individuals in the East Harlem area and prevent the spread of HIV / AIDS through community awareness events and program development and implementation.

Program Plan

The beginning phase which will happen cyclically over the course of one month is to raise community awareness by creating community awareness events. These events can range from community pot lucks to rallies. These are meant to bring people who need assistance into the program. Volunteers can help with these events and pass fliers.
