Several 'action plans' could address this problem. First of all, clinical community care-style settings staffed with nurses could dispense primary care, ideally at a subsidized cost. A cheaper 'per item' cost might actually be more financially feasible for largely uninsured communities, than paying to have the bureaucracy of health insurance and administrative staff.
The staff could be specifically trained in the needs of the community, whether this was primary care or agriculturally-related injuries. The clinics could offer some emergency care, or act as transportation hubs to hospitals when necessary. Doctors, in exchange for medical school tuition reimbursement, could staff the clinics, or doctors could make scheduled appearances at the clinic, on a rotating basis.
