Obviously, this is a concern both for patients and the organization; the former must be provided with care, especially in matters of urgency, while the latter has no function to perform if it cannot reach the patient. To address this issue in Kava, the target populations and their locations must be identified, existing infrastructure (roads, as well as plumbing and electricity for a new building) should be examined, and the various effect of floods, hurricanes, fires, etc. On this infrastructure -- as well as the likelihood/regularity of these events -- should be assessed in order to allow the organization to mitigate costs and damages while still providing access to care.

Other directly medical issues have been identified in the description of the island that are no doubt concerns of the Kava government and people as well as the World Health Organization and the international medical community at large. New cases HIV / AIDS can be greatly reduced with proper awareness campaigns and testing facilities, and new treatments also exist that can improve lifespan and quality of life for those suffering from the disease. Working with local organizations -- such as those identified by Alex -- and the government is essential to success in this area; plans must be developed that incorporate Kava culture instead of simply spreading a message from the newly-arrived Western medical team (IHI 2009).
The high risk of avian flu suggests a high risk of other communicable disease, which in many instances can be reduced down to a lack of adequate living environments and water supplies (IHI 2009). Before the organization can hope to have a large impact on the health of the population in Kava through direct medical care of individual patients, it must ensure clean living environments and practices among the people in their daily lives. Again, working with the government and local groups will not only ease some of the cost of these programs, but will make them far more effective in solving the problem and allowing the organization to perform its function (IHI 2009).

There are a host of additional problems both large and small that must be dealt with by the organization in their planned expansion, but careful planning and discussion with other relevant parties and stakeholders should allow for a successful growth of presence on Kava. Paradise might have been lost, but through careful planning and problem solving techniques it can be regained......
