Medical Procedures and Ethical Considerations

Randomized Clinical Trials (RCT) is a medical study whereby the participants are randomly assigned to groups or team which are then used to compare different treatments. The members of the groups are then subjected to a chosen treatment hence the group members will have similar treatments though different from members of another group. The same treatment of the members of a group is aimed at having objective results received from all the members of the group. However, the loyalty of the physician to the patients is compromised by using this method on the patients. In as much as it is the choice of the patient to participate in the trials, the random selection into the groups does not give the individual the prior knowledge of the kind of treatment that he will be subjected to. He may give concept, but this is largely uninformed consent since he is not aware of the group he may fall into.

The concept of equipoise in clinical studies comes in a situation where there are two or more therapies or treatments to a given disease or condition, yet none of them has been proven to be the best or superior in terms of the safety, efficacy, or even clinical usefulness.

This is what is referred to as clinical equipoise. The RCT as discussed above hence aims at disturbing the equipoise through the random trials to know which of the two or more interventions works the best among the identified population (Grady C., 2016). This concept of clinical equipoise then settles the ethical concerns of participants thinking that the medics involved in the study might intentionally manipulate some subjects to received the most unfavorable of the therapies.

Basic ethical principles

In the research involving human subjects, there are several guidelines that help in the protection of the human subjects involved. These guidelines can be summarized into three basic ethical principles; a). Respect for persons- this has two ethics involved in it, one being that the individual needs to be treated as an autonomous agent and the second is that, in the event that the autonomy is significantly diminished, then the individual is entitled to protection. The individual needs to be respected and allowed to make decisions on his own accord and those decisions need to be respected.
