Process and Outcome Measures for Each Disease Area

The number of members eligible for inclusion in the study area.

The plan identified 37,889 medical members eligible for the diabetes onset prevention project.

A member outreach effort will be conducted to 500 eligible Medicaid members to determine the interest in participating in the Diabetes Onset Prevention project.

The plan will seek to enroll, at a minimum, 104 members in the Diabetes onset prevention project.

Each group will have 26 subgroups

Part of the criteria used to select enrollees for each chronic disease arm and data collection procedures was using the data that was given by NYSDOH. The plan at first identified all Medicaid members that were diagnosed with pre-diabetes that did not have any kind of co-morbidity. Part of the criteria was to register 104 Medicaid associates. The plan employed a stratified random sampling methodology. In the study, the researchers picked its population set for the pre-diabetes examples along with its equivalent sub-groups. Each sub-group parallels to a detailed enticement arrangement. The arrangements are the following:

Reasons based on clinical milestone or result

Group C

Reasons based on both activities and result

Group D

No reasons (control group)

When going through the criteria each sub-group will have 26 contributors.
All of these 26 participants were pre-picked for enrollment constructed on their residential vicinity to participating providers. To prevent sharing of information concerning the reasons, each group had to attend distinct DPP provider programs and settings. The plan is meant to identify and then join with providers that provide the (DPP). The plan was then applied to the American Diabetes Association, American Hearts Association, New York City Quality and Technical Assistance Center (QTAC), Partnership for Prevention website and The Diabetes Prevention and Control Alliance (DPCA) for registers of service suppliers. Then the selection of program suppliers were based on their juxtaposition to Bronx and Brooklyn parts, accreditation status and resource volume.

The other part of the criteria used in this process involved the Plan and DPP providers. The criteria involved the coordination plus the collection and distribution of baseline and on- going statistics. By means of using the collaborative effort by the Plan and DPP suppliers, the following data was collected using the criteria that was and shared on a daily origin as decided by both the Plan and DPP suppliers. For enrollment identification some of the criteria used for each disease was Date enrolled into DPP program, Disease category, and the Incentive Assignment. Other criteria was the baseline weight, class assignment and the session date. Other criteria involved the weight for each session.
