Consider the case of Mr. a, the irrational quadriplegic who has little movement from the neck down, and lives in a nursing home. He is irrational in his behavior, and often demands very explicit forms of treatment, and then rejects or refuses these treatments or other treatments. He has become violent on occasion, and has created problems between patients and staff. He has been expelled from one nursing home, and may be from another. The home is bound to treat him, as he desires care and chose the nursing home. However, if he harms staff or other patients in any way, the nursing home is bound to provide a safe environment, and Mr. A is a detriment to the health and safety of others. In that case, the facility can refuse treatment for the good of the many, rather than the good of one patient. The author of the article notes, "Mr. a's care providers have an obligation to care for him in spite of his behavior, but they also have an obligation to protect other patients and staff from his actions" (Silvers, 2007).
