This case study highlights the importance of remote interdisciplinary collaboration in delivering comprehensive, patient-centered care for a 35-year-old female patient diagnosed with anxiety. Tapping into the specialized knowledge of different healthcare providers, including a primary care nurse, psychiatrist, therapist, and social worker, offers a comprehensive treatment plan that can be developed and implemented to improve the patient\'s outcomes and quality of life.Proposed Evidence-Based Care PlanBased on the patient\'s symptoms, medical history, and input from the collaborative team, the following evidence-based care plan is proposed:Pharmacological Interventions: As recommended by the psychiatrist, initiating medication therapy incorporating serotonin reuptake inhibitors (SSRIs) and benzodiazepines can effectively reduce the patient\'s anxiety symptoms and improve her overall well-being. SSRIs, such as sertraline or paroxetine, are considered initial treatment options for anxiety disorders due to their effectiveness and favorable side effect profile (Melaragno, 2021). For short periods to alleviate intense anxiety symptoms, benzodiazepines like clonazepam or lorazepam may serve as supplementary treatment (Dubovsky & Marshall, 2022).Cognitive-Behavioral Therapy (CBT): The therapist\'s recommendation of conducting remote CBT sessions aligns with evidence-based guidelines for the treatment of anxiety disorders. CBT is a well-established psychological intervention that helps patients identify and modify maladaptive thought patterns and behaviors contributing to their anxiety (Salkovskis et al., 2023). Methods like cognitive restructuring, exposure therapy, and relaxation training are effective in alleviating anxiety symptoms and enhancing coping abilities.Stress Management and Psychoeducation: The social worker\'s recommendation to provide education and counseling on stress management techniques, self-care, and social support networks is crucial for the patient\'s overall well-being.
