Diabetic Intervention Planning

The intervention plan that I would create and attempt to implement to address the health promotion issue of Type 2 diabetes is one that is fairly standard for treating this malady. Quite simply, it would involve patients monitoring their blood sugar levels multiple times a day. Additionally, it would also require their self-administration of insulin (Ehrlich, 2013) in order to maintain patient-specific targets for their blood glucose values. This intervention plan would occur for a finite period of time -- for a one month period -- in order to properly evaluate its efficacy in helping patients achieve target objectives for maintaining acceptable and healthy blood sugar levels. Individuals would have a variety of options that they could select for injecting insulin. They could either choose to utilize an insulin pen, insulin pump, or the "long-acting basal insulin glargine" (Trask, 2014). Patients would be expected to measure their glucose levels a minimum of five times a day in order to inject insulin as needed to preserve their health.

From a formative evaluator perspective, there is equipment that medical personnel could utilize (and which I, as the one designing this intervention could access) to determine how well patients were adhering to this intervention.

Firstly, professionals such as certified diabetes instructors can download data from the blood glucose meters of potential patients to determine how many times they were checking their blood glucose levels. This is an important first step that could assess how well the intervention was working. If patients were not checking their blood sugar levels the minimum required times, then there would be little use in checking additional data. The summative assessment, of course, would be based on the results of the blood glucose data. Data that was close to, at, or under the levels of each respective patients' blood glucose values would indicate efficacy for such an intervention. From a formative perspective this data could be checked weekly. From a summative perspective, it could be checked at the end of the month to determine the overall effectiveness of this treatment.

Thus, the are multiple tools that are required for this implementation plan. Firstly, it requires the requisite equipment for patients to check their blood sugar level multiple times of day.
