First Discussion Post ResponseI agree that age alone is not an appropriate method of triaging patients. Comorbidities, as you note, are also an important component of this. An additional observation is the fact that the use of a ventilator itself can be very taxing upon the physical system of an individual who is older, or who has significant respiratory comorbidities. Prioritization can be very difficult. But it is also important to keep in mind systemic inequalities and try not to replicate them. White & Lo (2021) note: “ICU triage policies should simultaneously promote population health outcomes and mitigate health inequities,” which means that a Save the Most Lives approach or even the Life Cycle approach is not always adequate (p.287). Individuals from historically marginalized groups may have poorer access to healthcare, and due to longstanding health inequalities, and often have a higher risk profile (such as having asthma, COPD, or a history of smoking).
