They can also increase the center's reach, by helping it to serve more patients without necessarily expanding its physical footprint. This helps also to improve access to care and promote better health outcomes.Plus, the center can tap into additional resources and funding opportunities via collaborations with local organizations. Together they can address the social determinants of health and thus try to reduce the long-term costs associated with treating chronic conditions (Park et al., 2020).
